Release of Liability and Terms of Agreement and Conditions for all Services Provided by Daisy’s Doggie Daycare, LLC
This signed document is an agreement between Daisy’s Doggie Daycare, LLC and the undersigned for any services provided by Daisy’s Doggie Daycare, LLC. Daisy’s Doggie Daycare, LLC agrees to provide dog boarding and grooming services.
1. Client understands this contract and takes responsibility for prompt payment of fees. Client shall pay any and all expenses and attorney fees for collection of past due accounts.
2. Daisy’s Doggie Daycare, LLC is authorized to seek any medical care if deemed necessary with release from all liabilities related to transportation, treatment, and expenses, and is authorized to approve medical and/or emergency treatment (excluding euthanasia) as recommended by a veterinarian. Client agrees to reimburse Daisy’s Doggie Daycare, LLC for any expenses incurred attending to this need. Client also authorizes Daisy’s Doggie Daycare, LLC to utilize an alternative veterinarian in the event the primary veterinarian is unavailable. Every effort will be made to contact Client prior to obtaining emergency care.
3. Any items and belongings owned or rented by Daisy’s Doggie Daycare, LLC in which are loaned to Client will be the responsibility of Client. In the event that the item(s) and/or belonging(s) of Daisy’s Doggie Daycare, LLC were to be stolen, damaged, and/or misplaced, Client will be responsible for fully reimbursing Daisy’s Doggie Daycare, LLC.
4. Client is responsible for any costs/payments due to bites or any other injuries. Clients will be responsible for any medical expenses and damages resulting from any injury to the dog trainer or other person by the pet(s). Client agrees to indemnify and hold harmless Daisy’s Doggie Daycare, LLC in the event of a claim caused by any person injured by the pet(s). If a bite occurs, Daisy’s Doggie Daycare, LLC is responsible for reporting it to the authorities. Daisy’s Doggie Daycare, LLC will not be liable for the injury, disappearance, death or fines of any pet(s) while performing any services.
5. All dogs are required to be vaccinated and be licensed by their city. Client attests to the fact that all licenses and vaccinations required by their state, the city in which they reside and/or the county of residence are current according to law.
6. Client releases Daisy’s Doggie Daycare, LLC from any all liability or responsibility for any damage to the Client’s property, or that of others, caused by Client’s pet(s) during the period in which they are in its care.
7. Daisy’s Doggie Daycare, LLC reserves the right to deny service or terminate service.
8. It is understood by the Client that there are no warranties or guarantees in connection with the services provided by Daisy’s Doggie Daycare, LLC.
9. Client waives all claims against Daisy’s Doggie Daycare, LLC and shall indemnify and hold Daisy’s Doggie Daycare, LLC’s officers, agents and employees harmless from any and all liability or claims from any death, bodily injury, personal injury, or any lost property that may arise out of or in any way be connected with the dogs training.
